We are looking to appoint an experienced Practice Nurse to join our expanding team. We are a friendly, stimulating and supportive Practice with a strong ethos of learning and development.
We have 11 Partners and 10 Associate GPs, 4 ANPs and a team of 8 Practice Nurses and 5 HCAs working out of our four sites at Windhill, Saltaire, Cottingley and Baildon. We have 24,000 patients and a diverse patient demographic.
We would like you to have experience in child and adult immunisations, dressings and wound care, cervical smear taking and chronic disease management. We would ideally like you to have qualifications in diabetes and asthma care.

Main duties of the job
Providing general nursing support to the GP and Nursing Team at the Practice. Experience of Long Term Condition Management is essential.
Job responsibilities
Assist in and perform routine tasks related to patient care as directed by Clinical leads, senior nursing staff and GPs. Such tasks include:
1. Basic wound care / ulcer care
2. Removal of sutures
3. Cytology
4. Urinalysis
5. Processing of samples
6. ECGs
7. Asthma/Peak flow readings/Spirometry
8. Venipuncture
9. Blood pressure monitoring
10. Ear Syringing
11. Routine immunisations including childhood immunisations
12. Chaperoning and assisting patients where appropriate who are being examined by another clinician.
13. Requesting pathology tests, for example urine culture, swabs
14. Long term condition management
15. Following agreed clinical protocols with referral to Clinical Leads, senior nurses or GPs as appropriate.
Other tasks will include:
1. Clean and sterilise equipment as appropriate.
2. Maintaining and cleaning equipment used by nurses and GPs
3. Maintaining GP and Nurses rooms, stocking, and rotating items as required
4. Participation in health campaigns, e.g. flu campaign.
5. Maintain general tidiness and cleanliness of nurses and treatment rooms
6. Maintaining and ordering health promotion literature.
7. At all times there will be a need to maintain accurate records
8. Ordering of vaccinations to maintain stock levels
9. Participate in administrative systems in the Practice
10. A duty to advise clinical leads and the Business Manager of potential problems or errors within the range of assigned tasks
11. Attend and participate in any Practice meetings when required.
12. On occasion there may be a requirement to undertake home visits
13. Any other delegated duties appropriate to the post

Person Specification
Qualifications
* Registered Nurse
* Minimum 2 years post registration experience
* In possession of two clinical Diplomas, e.g. respiratory, diabetes, cytology.
* Evidence of appropriate knowledge base
Experience
* Experience of working in accordance with the NMC Code of Professional Conduct
* Experience of working in primary care environment
* Experience of Microsoft Office software
* Experience of multi-skilled approach to Chronic Disease Management
* Understanding of clinical coding in general and Read coding in particular
* Experience of working in a complex organisation
* Experience of diagnosis and treatment of minor illnesses
* Evidence of involvement in Research and Development
* Experience of working with new technologies in health care practice
