Main duties of the job The post holder will work within an integrated 0-19 Universal Health Visiting and School Nursing Team within the Community. After a period of preceptorship, you will be responsible for the assessment of child and family health and development within the Healthy Child Programme, as well as the implementation and evaluation of other packages of care. You will provide packages of care to individuals, families and groups within the community following delegation from the Health Visitor and School Nurse Specialist Public Health Practitioner. As a member of the team you will be required to support in the co-ordination and delivery of client contacts, striving to achieve the activity targets specified within the commissioned Community Contract. As a member of the team, you will ensure that effective communication is maintained at all times and provide support for junior staff.
