The Healthy Families Team deliver Universal and targeted health services to the 019-year population in Nottinghamshire. You will be responsible for supporting the day-to-day management of a defined Caseload, working within a skilled and mixed team. This is an excellent opportunity for a suitably qualified, experienced nurse with sound communication skills to develop their role within the community setting. As part of the Healthy Families Team, you will be responsible for the delivery of public health and universal and targeted interventions across the 0-19 years age range; This will be delegated to them by the Specialist Public Health Practitioner for a defined population. This will be delivered in line with the National and Local guidelines and public health priorities. You will be is responsible for supporting and clinically supervising the skill mix team allocated to a defined geographical population to deliver and maintain high quality professional standards of practice. The team will work proactively using a partnership approach to promote health and contribute to tackling health inequalities within the level of competencies required for the post. You will work with children, young people and families within the home environment, school, or other identified community setting. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
