Summary
The role of the Practice Nurse is anevolving one and it is anticipated that the post holder/s will be grow to meetand develop beyond the description below.
Toprovide nursing services to the practice population by evidence based, qualityfocused care. To assess, plan, implementand evaluate care. To ensure clearchannels of communication exist between clinical, administrative and communityteams. To project a positive and friendlyimage to our patients and other professionals.
Teamworkis a priority within the practice and there may be times you will be asked toassist or undertake other duties; these will always be within your level ofcompetence. Accordingly, there may be from time to time, a request to providecover for sickness or absence.
Principal Responsibilities
To provide assessment, treatment, screening and healtheducation services and advice, working in accordance with set protocols,guidelines and NMC Code of Conduct.
Provide nursing service/treatments to patientsindependently or in participation with general practitioners with reference toaforesaid agreed protocols and guidelines.
ProfessionalResponsibilities:
Tobe qualified and a member of the NMC
Maintaincontinued education by attendance at courses and study days relevant/useful ornecessary for personal/professional development including attendance at thelocality protected learning time (PLT) events.
Tohave the ability to work on own initiative, acknowledging limitations ofoneself and in the delegation to others. Recognising when referral to otherhealth professionals is needed.
Tobe familiar with changes and progress in nursing care.
Evaluatingand adapting care using evidence based, best practice, in consultation withcolleagues as a forum for team education, development and audit.
Usingevidence based, best practice to assist in the formulation of practicephilosophy, strategy, and policies.

Specimencollection and testing
1. Home visits for Housebound patients to include routine checks (eg Diabetic) and flu vaccines.
2. Referrals Podiatry, Counselling, Audiology, etc
3. To maintain accurate, comprehensive records of all consultations and treatments in the patients notes, both written and computerised, awareness of QOF, read codes and alerts needed for standardised care.
4. All associated nursing administration, Vaccine forms, Travel Health Questionnaires, protocol review and assisting Reception with queries
5. Manage clinics effectively and contact patients to attend.
6. To take personal responsibility for own personal/professional development, providing sufficient evidence of the achievement of set objectives through individual performance and the review process.
7. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
Attendand participate in practice and staff meetings as appropriate
Totake prompt action to defuse situations of complaints and be equipped with theknowledge to advise patients and relatives/carers about the complaintsprocedure.
Maintainappearance and behaviour in keeping with a member of the nursing profession andin accordance with codes of conduct.
Supportand co-ordinate health promotion campaigns,, flu, stop smoking, healthylifestyles, etc
Maintaineffective liaison with other agencies and staff concerned with patient care andwith other disciplines within the practice, with appropriate regard to confidentiality.

Assistingwith Coil and implant appointments

Prescribing
Participatein the education of students of all disciplines.
Tobe responsible for the efficient running of the treatment room and equipmentand to ensure the clinical areas are always safe for staff and patients.
Toadhere to strict Infection Control in line with guidelines and policies.
Advisepatients and relatives/carers on general health care and ailments with referralto GPs as necessary.
