This is an exciting opportunity for a Care Co-ordinator to work with the Neston and Willaston Care Community Team. You will be a central co-ordination point for local multi-disciplinary activity, ensuring high quality customer service.
You will work alongside the Care Community Team and partners including local authority GPs and the voluntary sector, to support patients within the care community.
This post is part time: 15 hours per week.
Main duties of the job
1. Good interpersonal and communication skills.
2. Co-ordination of multi-disciplinary meetings.
3. Management of daily meetings.
4. Liaising with multi-disciplinary team members.
5. Signposting team members and patients/carers to relevant services.
Working days are Monday and Tuesday 09:00 - 17:00.

Qualifications
* Able to demonstrate a good command of English language, verbal and written.
* Knowledge of Office Administration/Customer services acquired through training or experience to NVQ level or equivalent qualification.
Knowledge and Expertise
* Knowledge of Health and Social Care administration and processes.
* Knowledge of Microsoft office applications, internet and email.
* Knowledge/familiarity with medical terminology.
Experience
* Able to work well in a team.
* Experience of working in an Office Environment.
* Experience/knowledge of issues of working with confidential information.
* Experience of working with healthcare professionals/previous experience in the NHS.
* Working in a busy and demanding environment.
