Come and join us An exciting opportunity has arisen for a qualified Band 6 Social Worker to join our dynamic multi-disciplinary Specialist Mental Health Service for Older adults serving the needs of older people within the population East Hertfordshire covering Broxbourne Borough, East Herts District, and Welwyn Hatfield Borough. You will be responsible for the delivery of care to an allocated caseload of service users experiencing, or recovering from, mental health difficulties. You will also be completing holistic assessments, developing innovative support plans, reviews and CPA in conjunction with other professionals while building networks, raising awareness in the area and developing valuable links for the team. You will be working alongside Consultant Psychiatrists, Psychologists, Nurses, Occupational Therapists and Support Workers to achieve the best outcomes for the individuals you work with. You will provide direct support and advice to the service users, their families and carers, within the recovery framework, embracing the individual right to choose and control.
