Job overview
Harrogate District NHS Foundation Trust is looking to recruit a Clinical Service Development Lead to support the delivery of our Urgent Adult Community Care services. The role includes support to the following services:
· The Urgent Community Response service
These two services were commissioned to enable patients to avoid hospital admission or access earlier discharge in order to reduce the harm that arises from unnecessary hospital attendances and admissions. Both services provide a timely response to referred patients, either in hospital or in the community, to deliver appropriate medical and health interventions, which allow the patient to receive treatment in the most appropriate place for their individual needs.
Main duties of the job
This post will initially be dedicated to supporting the services work towards fuller integration through improved development of skills and competences, reduced carve outs, and the creation of safe, effective and streamlined pathways for patients accessing the services.
The successful candidate must be comfortable with potential change of focus as the services move forward and be in a position to support and lead change in line with current clinical evidence, digital innovation and local and national standards and key performance indicators.

Qualifications
* Relevant Masters degree in Advanced Clinical Practice
* Registered as non-medical prescriber with the NMC/HPC
Experience
* Significant post registration experience in the speciality area or a related area where advanced skills could be transferable
* Experience of leading evidence service improvement and innovation through service/practice development initiatives, audit or research
Knowledge
* Knowledge of local and national healthcare agendas/strategy and policies and how they relate to the specific service
You must have appropriate UK professional registration.
