We are looking for an experienced doctor who is comfortable working within a strong multidisciplinary team to coordinate care for people with complex needs in a community hospital setting .

Assessment of new admissions to the unit and timely completion of discharge documentation
Review of clinical deterioration in patients at the guidance of nursing staff or medical colleagues
Supervise trainees and Trust Doctors in your clinical areas
Supporting the Senior Doctor team with ward rounds and MDTs – including covering these in event of absence.
The post-holder will work as a member of the multi-disciplinary team, ensuring patient care is based on need with services that are accessible and acceptable to patients and carers. They will be available by appointment to meet with family members and other carers when appropriate to discuss patients’ management and progress
Inclusion on the resident and non-resident on call rota based at Mount Gould Hospital. When non-resident on call y ou are required to be able to attend site within 30 mins of being contacted or complete your on call as resident on call.

The post-holder will be expected to uphold the principles of clinical governance and to adhere to the organisation’s clinical governance requirements as outlined in Standards for Better Health. You are expected to be actively engaged in the organisation’s clinical governance programme and regularly attend governance meetings.
Your base will be Mount Gould Hospital (MGH)
The right person for the job will have experience working with older people living with frailty, and ideally experience in community hospitals or rehabilitation .

There are 67 beds across these two community hospital wards for geriatric and post-acute rehabilitation. The case mix is broad and includes those following orthopaedic surgery, trauma and amputation as well as acute physical illness in older people. The majority of patients are admitted from Derriford Hospital following an acute episode of care. There is an experienced multidisciplinary team across these wards including nursing, physiotherapy, occupational therapy, pharmacist, social worker, dietician and psychiatric liaison nurse support .
Following a successful pilot we have up to 8 End Of Life beds in side rooms across the two wards which are supported by the palliative care team.
Out of hours cover is provided through the senior nurse on site, supported by off-site hospital wide on-call junior doctor and senior on-call rota (in partnership with Livewell Southwest [LSW]) .
There are 6 nursing and medical teams across the two wards. Each patient is seen weekly by a senior doctor, and there is a weekly MDT to discuss all patients. You will be responsible for the medical care for patients across the two wards. You will cross cover absent colleagues too.
