An exciting opportunity has arisen for an enthusiastic, well-motivated and flexible Nurse Practitioner to join our Urgent Community Response Services across Sunderland within the Recovery at Home.
The team consists of an extensive MDT which includes Nurse Practitioners, Nurses, Occupational Therapists, Physiotherapists, Health Care Assistants, Therapy Assistants/Exercise Practitioners and Administrative staff. The team also works closely alongside partner agencies including those from the local and voluntary sector.
This role would involve assessing people in their own homes who are either acutely unwell or have recently been discharged from hospital and require enhanced intervention and short-term support for their physical and functional health needs to remain at home.
The service is fast-paced, and you would be expected to see a wide range of patients who present with a variety of conditions.
This is a unique and rewarding role which will allow you to work with patients within their usual place of residence to enable them to remain there safely alongside you developing your own clinical skills.
Full or part-time hours will be considered.
Main duties of the job
The service is committed to providing a sound environment for education and training to support the successful candidate within this role.
The post holder will need to be proactive with their own learning and work to identified timescales.
* The post holder will take a lead role in The Recovery at Home Team, providing care to people in their own homes to prevent admission/readmission to hospital and also supporting early discharge from hospital by providing rapid assessment of patients in the community over a 24 hour period.
* To work autonomously, assessing individual needs, initiating investigations, determining a differential diagnosis and initiating appropriate holistic, evidence-based treatment and care. Refer patients who require medical intervention to the appropriate services.
* Undertake triage within the service determining clinical priority of calls working with the coordinator and GP ensuring teams are dispatched to respond to the referrals.
* To prioritise and delegate work appropriately, monitoring and ensuring safe working practice.
Person Specification
Qualifications
* 1st level registered nurse.
* Degree in Nursing.
* Non-Medical Prescribing qualification or due to complete within next 6 months.
* Portfolio of evidence of competence and continuous professional development.
Experience
* Significant experience of working at a senior level caring for the acutely unwell/deteriorating patient.
* Experience of leading clinical development.
* Experience of managing change and team of staff.
* Multidisciplinary and multi-agency working.
* Previous experience of work as a nurse practitioner or working in an acute setting.
Skills and Knowledge
* Excellent communication and organizational skills.
* Extended nursing skills.
* Demonstrable leadership skills.
* Ability to work under pressure and achieve tight deadlines in a complex/changing environment.
* Clear concise record keeping/report writing skills.
* Range of IT Skills.
* Community experience.
