Job summary
To work as anautonomous practitioner, responsible for the delivery of total patient care, workingas part of the practice multidisciplinary team, delivering care within theirscope of practice to the entitled patient population. The Role will encompassworking across four of the embedded sites within the Heart of Lincoln remit.
The NursePractitioner / Advanced Nurse Practitioner / Advanced Clinical Practitioner is expected to, within their professionalboundaries, sees patients on an appointment basis both on the day and pre planned care appointments, providescare including initial history taking, clinical assessment, diagnosis,treatment and evaluation of care. They will demonstrate safe, clinicaldecision-making and expert care, including assessment and diagnostic skills,for patients presenting at the practice. They will work collaboratively withthe general practice team to meet the needs of patients, supporting thedelivery of policy and procedures, and providing nurse leadership as required.

Job responsibilities
a.Managepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice to patients
b.Implementand evaluate individual specialised treatment plans for chronic diseasepatients where trained and skilled to do so.
c.Identify,manage and support patients as risk of developing long-term conditions,preventing adverse effects on the patients health
d.Provideadvanced, specialist nursing care to patients as required in accordance withclinical based evidence, NICE and the NSF
e.Requestpathology services as necessary
f.Processand interpret pathology and other test results as required
g.Providedaily clinics, delivering patient care as necessary, referring patients tosecondary / specialist care as required
h.Maintainaccurate clinical records in conjunction with extant legislation
i.Ensureread codes are used effectively
j.Maintainchronic disease registers in accordance to QOF
k.Chaperonepatients where necessary
l.Prioritisehealth issues and intervene appropriately
m.Supportthe team in dealing with clinical emergencies
n.Recognise,assess and refer patients presenting with mental health needs
o.Extendedand supplementary prescriber, adhering to extant guidance
p.Supportpatients in the use of their prescribed medicines or over the counter medicines(within own scope of practice), reviewing annually as required
q.Contributeto practice targets (QOF etc.), complying with local and regional guidance
r.Liaisewith external services / agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)
s.Delegateclinical responsibilities appropriately (ensuring safe practice and the task iswithin the scope of practice of the individual)
t.Supportthe clinical team with all safeguarding matters, in accordance with local andnational policies
u.Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately

Essential
1. Registered Healthcare Professional Nurse or Paramedic
2. Qualified Independant Prescriber V300
3. Qualified Triage Nurse/ Clinician
4. Minor Illness Qualification
Desirable
5. Post graduate diploma or degree (Advanced Practice Qualification) Or working towards this.
6. As per JD and personal Spec
