Lead Pharmacy Technician – Medicines Administration and Optimisation

Working as part of a multi-professional disciplinary team and providing a holistic oversight of medicines management processes undertaken in adult physical health district nursing practice. Supporting, teaching, leading, managing and providing advice and support regarding best practice guidance in reference to medicines optimisation and medicines management. Aim of the post is to optimise patient care and ensure that where possible that patient independence is preserved and maintained.
Main duties of the job
The post holder will be responsible for their designated workload within their speciality area with access to the support, advice and management from a senior clinician whilst not directly supervised. In doing so, they will assess, plan, deliver and evaluate patient centred programmes of care ensuring that they practice within their sphere of competence and knowledge. The post holder will work to professional protocols and regulatory body codes, standards and guidance always ensuring that their practice is grounded in the best available evidence based theoretical and practical knowledge. The post holder will provide professional leadership and management of medicines management within community and district nursing service. They will line manage and supervise specialist pharmacy technicians.
Duties are varied but will include:
* To undertake medication administration
* To screen new patient referrals under the district nurses teams, contacting GPs and / or other healthcare providers to ascertain the patient's medication history and advising Trust staff of identified discrepancies.
* To identify potential pharmaceutical issues that may arise when patients are reviewed and work with the multi-disciplinary teams to resolve these queries.
* To be responsible for medicines management for patients within the community

Person Specification
Qualifications
* Registered with the General Pharmaceutical Council (GPhC)
* Completed technician accuracy checking (either hospital in house, or accredited qualification)
* Leadership, management and quality improvement experience and/ or qualification.
* Responsive, proactive approach to ongoing professional development aspiring to attain the highest levels of practice - personally and with regard to others.
Training
* Evidence of regular Continuing Professional Development.
* Awareness of performance measures (specifically with reference to Clinical Governance and Healthcare Standards).
* Computer skills including the use of Pharmacy software.
* Health & Safety / COSHH issues.
* Experience in teaching/tutoring
Experience
* Experience as working as a Medicines Management Technician
* Appropriate demonstrable post-qualification Pharmacy work experience, covering the main skills areas, and including the relevant speciality.
* Able to assess staff performance against agreed standards and criteria
* Experience of working to standard operating procedures
* Experience in service development and implementation of new policies/services
* Relevant professional, managerial and leadership experience of pharmacy technicians and support staff
* Experience of medicines optimisation
* Excellent time management skills required to manage own time most effectively by planning and prioritising workload appropriately.
* Use judgement to respond to service/ staffing changes in order to maintain services and ensure a safe working environment within the team, also regarding when to escalate any concerns
* Experience in dealing with discipline and performance management
* Experience in dealing with short- and long-term staff sickness
* Experience of HR processes - recruitment, performance management, employee wellbeing and managing attendance.
Knowledge
* Knowledge of medicines management
* Evidence of continued professional development in line with GPhC registration.
* Governance frameworks and risk management.
* Understanding of health services and medicines/ pharmacy related issues in primary and secondary care.
* To have knowledge/ experience of quality improvement and service development
* Knowledge and understanding of pharmaceutical technical procedures specific to the area of work
* Understanding of Information Governance and Confidentiality
* Understanding of equality and diversity issues and how this affects patients, visitors and staff
* Understanding of what the NHS Constitution means to you, and your responsibilities to the public, patients and colleagues.
* Experience as working as a medicines administration technician
* Accurately record all issues, updating the patient records on SystmOne
* Use of Health Roster and experience in dealing with annual leave requests
* Knowledge of electronic prescribing
* Understanding of local and national priorities through thorough knowledge of the NHS, government and local structure, policy, process and guidelines relevant to medicines management, pharmacy and mental health.
