An exciting opportunity has arisen for 2 posts based within our Community Children's Nursing service.
You will be supporting children and young people with complex health needs within their homes and across various settings. You will be a RN (child) /RSCN who has had previous experience working with children with complex health needs.

Main duties of the job
* To provide high quality nursing care to children and young people.
* To be responsible for a delegated workload as agreed with the Team Lead and senior Community Children's Nurses.
* To provide nursing care, advice and support in negotiation with the child and family /carers to meet their individual needs.
* To assess care needs, plan implement and evaluate individualised programmes of care to meet identified needs in negotiation with children, young people and their families.
* To manage a nursing caseload with the support of senior nurses.
