Job summary An exciting opportunity has arisen for a full time band 4 - 12 months secondment as a Patient Pathway Coordinator working within the Children Hospital Unit. The successful candidate will be required to work closely with both the clinical and administration team to ensure patients move seamlessly and safely through the limb recon and bone infection pathway, without delay. As patient pathway coordinator, you will be responsible for not only tracking patients referred to the service against key performance indicators but also escalating issues in the pathway that may cause a delay to the patients timely treatment. Furthermore, the successful candidate will be responsible for providing secretarial support to the Bone Infection team and will be responsible for booking appointments, typing clinic letters and monitoring the patient pathway with expert precision. Excellent communication, gold standard organisation and interpersonal skills are key for this role as there is a need to communicate complex and sensitive information and provide support to both clinical and administrative staff. This is a busy and dynamic role with a varied and interesting workload and would suit someone that has excelled attention to detail and is confident at problem-solving. Main duties of the job To be the main point of contact for all patients issues relating to their pathway of care. To be responsible for the management of the patient pathway, from referral to first outpatient appointment, through diagnostics, treatment planning, treatment, and follow-up, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process. To have a sound understanding of 18 week waiting time targets, rules and procedures, and to work proactively to ensure that all targets are maintained.
