This is an exciting opportunity to join the Adults Area Team for a three-month role, offering a competitive hourly rate of £31.27. Based at Pershore Day Centre in Pershore, you will work full-time hours from 9 am to 5 pm, Monday to Friday. What you will do:
1. Conduct assessments and reviews of adults, focusing on their individual needs and wellbeing to craft tailored care plans.
2. Work as a part of a collaborative team to provide high-quality services that promote independence and improve the quality of life for service users.
3. Facilitate access to community resources and services, ensuring clients receive comprehensive support for their diverse needs.
4. Participate in multi-disciplinary meetings, sharing insights and expertise to support holistic care approaches.
5. Maintain accurate and timely records of all interactions, ensuring compliance with both local and national statutory requirements.
Requirements of the Social Worker:
1. Degree or equivalent in social work
2. Current Social Work England registration
3. Significant frontline experience in Social Services within the UK working as a qualified Social Worker
