Reporting to the Director of Specialist Services, the Physiotherapist will be responsible for assessment and treatment of adults in their own homes during their Reablement journey, with the expectation to work within Norfolk and Waverly ICB. The role includes providing physiotherapy advice, guidance, and information to service users, support workers, and health and social service professionals.
Main duties of the job
1. To assess, plan, implement, and monitor physiotherapy treatment programmes for Reablement adults within their own homes across all specified areas of Norfolk.
2. To analyse, evaluate, and modify treatment in order to measure progress and assure effectiveness of the intervention.
3. To be professionally and legally accountable for all aspects of your own work to ensure a high standard of clinical care.
4. To refer and escalate to other staff and agencies within Cera and key external stakeholders as appropriate.
5. To assist in relevant aspects of health promotion for service users within the Reablement pathway.
6. To refer to and recommend the provision of aids to assist the service user to achieve their reablement goals.
7. To establish effective communication networks with service users, support workers, and other team members.
8. To meet company set targets.
9. To be responsible for the effective and efficient organisation of your own time management, prioritisation, workload, and the safe use of equipment.
10. To adhere to Cera's rights-based approach, taking into account service user choice.
11. To take reasonable care for ensuring your own personal health and safety and that of service users and other staff that may be affected by your acts or omissions.

Job responsibilities

You will manage your time, workload, and equipment efficiently, meet company targets, and adhere to Cera's rights-based approach, prioritizing service user choice. Additionally, you must ensure the health and safety of yourself, service users, and colleagues.

Qualifications
* Professional Qualification in Physiotherapy
* HPC Registration
* Two years experience working as an autonomous Physiotherapist
* Experience in working in the Community
* Experience of team working and liaison with other staff groups
* Evidence of CPD portfolio
* Awareness and understanding of NICE guidelines
* Experience working in a Reablement environment to achieve personalised goals
* Experience working with a wide range of service users with varying conditions and needs
