Weare looking for a First Contact Physiotherapist to provide clinical expertiseand make decisions about the best course of action for patients' care.
As a first contact practitioner (FCP), acting as afirst-contact physiotherapist, you will be providing initial assessment,diagnosis, and treatment for patients in need of musculoskeletal (MSK) relatedcare. As the first point of contact for patients, this individual plays apivotal role in ensuring timely and effective care delivery.
Based in Northgate Health Centreand working mainly across two of our four practices in the PCN, you will offerremote and face to face appointments. If you are a motivated self-starter andpassionate about offering excellent care, we would love to hear from you.

Main duties of the job
The purposeof the role is not to deliver physiotherapy treatment for patients in place ofthe community and hospital outpatient service. The role intends to shiftworkload from GPs, by providing patients with an appropriately trained andcertified healthcare professional to assess and manage their MSK presentations.This will include seeing patients to establish a rapid and accurate diagnosisand management plan.
Progress andrequest investigations to facilitate diagnosis and choice of treatment pathway;using diagnostic services such as x-rays and blood test. Interpret and act onresults to aid diagnosis and the management plans of patients.
Providepatient self-management advice, in ways that facilitate behavioural change,optimise individuals' physical activity, mobility, fulfilment of personal goalsand independence, and that minimise the need for pharmacological interventions.

Job responsibilities
1. Take professional responsibility as afirst-contact physiotherapist, with high-level decision-making andclinical-reasoning skills to assess, diagnose, triage and refer patients foronward treatment.
2. Support GPs with management ofcomplex caseloads (including patients with long-term conditions, co-morbiditiesand multi-factorial needs).
3. Contribute to development of servicedelivery.
4. Accountable for decisions and actionsvia HCPC registration, supported by a professional culture of peernetworking/review and engagement in evidence-based practice.
5. Ensure care is proactive, preventivein focus and population based, with an emphasis on early intervention.
6. Provide care which is tailored to theindividual needs. This would include, appraising the impact of individuals'clinical status on their general health, well-being, employment status(including in relation to function, physical activity, mobility and independence).
7. Through patient assessment andworking in partnership with patients and their carers, make decisions about thebest pathway of care, informed by the urgency and severity of patient need,patient acuity and dependency, and the most appropriate deployment ofresources.
8. Support patients to set their owngoals and be confident in their approach to self-management.
9. Communicate effectively andappropriately with patients and carers complex and sensitive informationregarding diagnosis, pathology and prognosis.

11. Provide learning opportunities forthe whole multi professional team within primary care at network of Practicelevel.

13. Manage interactions in complexsituations, including with individuals with particular psychosocial and mentalhealth needs and with colleagues across the primary care team, sectors andsettings.

Qualifications
Essential
1. -Undergraduate degree in Physiotherapy.
2. -Health & Care Professions Council (HCPC) Registered Physiotherapist.
3. -Member of the Chartered Society of Physiotherapy (CSP) or appropriate professional body.
4. -Completed Level 7 Modules in MSK related areas of practice (advanced assessment / diagnosis / treatment).
5. -Evidence of continuing professional development in relevant clinical and management areas.
6. -Evidence of completion of Stage 1 of the FCP roadmap ( the knowledge/skills and attributes).
Desirable
7. -A Masters Level qualification or equivalent specialist musculoskeletal knowledge, skills and experience.
8. -Postgraduate FCP Module.
9. -Clinical supervision / mentoring qualification.
10. -May hold or be working towards a prescribing qualification.
