This is an exciting 12 month secondment / fixed term role working 22.5 hours per week for a Discharge Facilitator based at Haytor ward.
Haytor Wards multi-disciplinary team provide a service for adults with a mental health illness. We are a busy, motivated and interesting team, who value working together. The Clinical Team Leader role is essential in supporting the team to achieve its aim.
Main duties of the job
1. Develop working relationships with service users, carers/family members to facilitate discharge from hospital.
2. Establish firm working relationships with inpatient, community mental health team colleagues, and housing and support providers from public, private and third sectors.
3. Develop and accurately record comprehensive support plans, risk assessments and relevant reports to inform care management and social care processes.
4. Represent the Trust at meetings in an appropriate and knowledgeable manner including, on occasions, giving presentations using relevant media.
5. Communicate highly complex and sensitive information effectively across all levels within and outside of the organisation, to advocate collaborative working and promote best outcomes.
6. Contribute to Social Care funding panels in relation to providing information, advice or presenting applications.

Haytor ward provides acute inpatient care to adults experiencing mental health difficulties. The role of Discharge Facilitator has been developed to specifically work with inpatient and community colleagues as well as family members/carers and service providers to support smooth discharge pathways out of hospital.
Using care management techniques and a multi-disciplinary recovery oriented approach, the post holder will be expected to support individuals to access community resources and local provision to meet their needs in addition to brokering appropriate funded move on placements or support packages. This may also include services to support homelessness and dual diagnosis.

Essential
1. Experience of care management and working with people with mental health difficulties.
2. Experience of working with people with mental health difficulties in a community and inpatient setting.
3. Experience of multi-disciplinary/multi-agency working, particularly regarding housing, accommodation and support providers.
Qualifications
Essential
1. Educated to degree level or equivalent in: Health/Social Care or equivalent housing qualification or experience.
2. Registration with Professional Body if qualified healthcare professional.
3. Knowledge and understanding of the legislative framework around mental health, social care and housing, eg: Care Act, Mental Health Act, Mental Capacity Act.
