This post is for a qualified ACP with a passion for primary care within the community. You will bring your clinical expertise to a wider multidisciplinary team. Clinical support and supervision are available for all staff.
Main Duties of the Job
1. To be a clinical expert and senior clinician in the field of Primary Care.
2. To work with the PCN Lead ACP and wider team to continually improve the quality of care provided including the continuation of service improvement to demonstrate better outcomes for our patients.
Your Health Partnership (YHP) is a cutting-edge Primary Care Network working as part of Sandwell and West Birmingham Hospitals Trust. The successful applicant will need to share the same values of YHP and work as part of a large multi-disciplinary team including GPs, Nurses, Clinical Pharmacists, Allied Health Professionals, and Administrators.

Qualifications
* Registered with relevant professional body- e.g. NMC/HCPC
* Masters Degree in Advanced Practice
* Portfolio of evidence
* Independent non-medical prescribing, if from a core profession in which NMP is supported or when relevant to the role
* ILS/ALS/APLS/ATLS
Experience
* Extensive clinical experience
* Excellent clinical and practical skills
* Evidence of practice within the four pillars of Advanced Practice
* Evidence of continuous professional development and the practical application of new skills to the workplace
* Direct supervision of junior staff
* Experience of working within specialty

* Advanced interpersonal and communication skills with an ability to work effectively with diverse populations
