We've got an exciting opportunity for you as a Social Worker in Adult Social Care. Our mission is to empower young adults to live independently as possible, and we need your expertise to make it happen. This role will challenge you in a dynamic and diverse environment, helping you expand your skills and knowledge in the practice of social care. Your duties and responsibilities will include:
1. Conducting new Care Act compliant eligibility assessments for young people and their carers, to identify eligible social care needs and developing focussed, person centred and strengths based outcomes within the support plans.
2. Monitoring, reviewing and maintaining up to date support plans, ensuring they meet the changing needs of young people and reflect good value for money whilst advocating for young people's rights, choices, and access to services.
3. Safeguarding and promoting the welfare of young people, adhering to relevant legislation and policies, including being able to identify and assess levels of risk and need (within statutory frameworks) in complex situations and undertake relevant safeguarding investigations to deliver protective and/or supportive services for individuals at risk, including work that may be required by the Court of Protection where necessary.
4. Hold and effectively manage a complex and varied caseload with ability to make sound professional judgement with appropriate supervision, guidance, and support, reflecting the function of the team in accordance with policy, procedures, guidance and legislation.
5. Identify young people who have complex and/ or primary health needs and initiate the Continuing Healthcare Process to enable access to NHS funding for individuals who are eligible.
6. Collaboration with multidisciplinary teams, including children's services, education, healthcare professionals and community partners to enable a holistic approach to meeting the needs of the young person.
As a Social Worker within the Transition Team, you will have an essential role as the first point of contact for our young people in Sutton, supporting their journey into adulthood. You will begin to develop the relationship with the young person aged 16, and work in collaboration alongside them, their carers and colleagues in childrens, health and education services to ensure they have the required level of support to meet their needs.

Mental Health, Learning Disabilities & Transition
Within our boroughwide specialist service, there are three social work teams - The Transition Team, The Learning Disability Team, and the Mental Health Team.
The Transition Team in Sutton takes pride in supporting staff to learn and develop, with a comprehensive induction programme and access to regular training on a diverse range of topics. Staff have regular professional supervision which covers probation, performance and annual appraisals to focus on career development. Staff are able to work flexibly between home and office to meet the needs of the service, with a minimum requirement of 2 office based days per week, as we recognise the benefit of a hybrid model of working in managing work life/ balance.

About You
This position offers an excellent opportunity for individuals with outstanding communication skills and a compassionate manner, who are capable of effectively managing fast paced work. Candidates should have the experience and ability to work under-pressure in a fast paced environment, in order to manage competing demands. We encourage applications from candidates with experience in adult social care and those passionate about delivering person-centred care and prevention strategies to manage risk.
In addition, we would expect the following from all suitable candidates:
7. Qualified Social Worker with current registration with Social Work England and eligibility to work in the UK.
8. 2 years post qualifying experience working in adult services, assessing and supporting individuals with diverse needs and successful completion of their ASYE.
9. Sound knowledge of relevant legislation, including the Care Act and Mental Capacity Act as well as social work theories
10. Experience in Adult Safeguarding work
11. Strong assessment, outcome focussed care planning, and risk management skills
12. Ability to travel across the borough to visit individuals, occasionally being required to undertake visits out of borough
