Main duties of the job
You will have two years of experience in a General Practice setting with excellent interpersonal skills.
You will be required to undertake the full range of Practice Nurse Duties including cytology, baby immunisations, chronic disease management, and health promotion.
You will work with the full support of the practice team together with benefiting from the substantial networking opportunities across our other practices in the area. This is a great opportunity to join a well-established team.

The ideal applicant will have experience in the Quality and Outcomes framework and other targets required of modern-day general practice, confident with IT and management of processes in a rapidly changing environment.

We have a great network of Practice Nurses available within the area who are keen to share knowledge and help support and develop other members of the team together with an experienced Lead Nurse who is available for mentoring and support.
Job responsibilities
Responsible for the delivery of practice nursing care to the practice population. They will deliver evidence-based care for patients presenting with long-term conditions and provide preventative health care to the practice population. They will raise awareness among the population of the symptoms of long-term conditions. They will be committed to identifying and reducing barriers to presentation to primary care services. As an autonomous practitioner, the nurse is responsible for the care delivered, demonstrating clinical decision-making skills in the management of patients. They will work collaboratively within the practice team to meet the needs of patients, support the delivery of policy and procedure, and provide nurse leadership as required.
Clinical Nursing Practice
1. Assess, plan, develop, implement and evaluate programmes which promote health and well-being, and prevent adverse effects on health and well-being.
2. Assess, plan, develop, implement and evaluate individual treatment plans for patients with a known long-term condition.
3. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate.
4. Find cases of undiagnosed disease.
5. Undertake outreach activities within a range of settings within the target area.
6. Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug based treatment methods using a management plan, and in-line with national and local policies and practice needs.
7. Undertake agreed procedures which include but are not exhaustive: Blood pressure, pulse rate and rhythm, temperature, height and weight - body mass index, venepuncture, ECG, glucose tolerance test, peak flow readings, inhaler technique, spirometry, cytology, travel and childhood vaccinations, interpretation of blood results via the pathology link.
8. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiating effective emergency care.
9. Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care.
10. Deliver opportunistic health promotion using opportunities such as new patient medicals.
11. Provide information and advice on prescribed or over-the-counter medications, on medication regimens, side effects and interactions.
12. Review medication for therapeutic effectiveness, appropriate to patients' needs and in accordance with evidence-based practice and national and practice protocols.
13. Assess and care for patients attending with uncomplicated wounds.
14. Support and guide women attending for family planning advice.
15. Support and manage the needs of women attending for cervical cytology.
16. Recognise, assess and refer patients presenting with mental health needs in accordance with the National Service Framework (NSF) for mental health.
17. Implement and participate in immunisation programmes for both adults and children.
18. Advise, support and where appropriate vaccinate patients travelling abroad.
19. Promote and deliver evidence-based care for patients attending with aural conditions.
20. Assist senior practitioners in providing minor surgery.
Communication
1. Utilise and demonstrate sensitive communication styles, to ensure that patients are fully informed and consent to treatment.
2. Communicate and support patients who are receiving bad news.
3. Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background, and language differences.
4. Anticipate barriers to communication and take action to improve communication.
5. Maintain effective communication with individuals and groups within the practice environment and with external stakeholders.
6. As part of the outreach service; undertake liaison with patients' GPs.
7. Act as an advocate when representing the patients and colleagues' viewpoints to others.
Delivering a Quality Service
1. Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC).
2. Produce accurate and complete records of patient consultations consistent with legislation, policies and procedures.
3. Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
4. Deliver care according to the SSP policies, the NSF and National Institute for Clinical Excellence (NICE) guidelines.
5. Assess effectiveness of care delivery through self and peer review, benchmarking and formal appraisal.
6. Participate in the maintenance of quality governance systems and processes across the organisation.
7. Utilise the audit cycle as a means of evaluating the quality of the work of self and team, implementing improvements where required.
8. Assess the impact of policy implementation on care delivery.
9. In partnership with other clinical teams, collaborate on improving the quality of healthcare, responding to local and national policies and initiatives as appropriate.
10. Evaluate the patients' response to healthcare provision and the effectiveness of care.
11. Support and participate in shared learning across the practice and the wider organisation.
12. Identify learning needs from patient complaints, clinical incidents and near misses utilising a structured framework.
13. Participate in the performance monitoring of the team, providing feedback as appropriate.
14. Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of the statutory health procedure and local guidance.
15. Work within policies relating to domestic violence, vulnerable persons, substance and alcohol misuse; and refer as appropriate.
