Mental Health Liaison Service provides assessments and interventions for patients across the age range 18 The successful applicant would be part of a specialist multi-disciplinary team supporting patients within hospital settings who are experiencing co-existing mental health and physical health problems. Main duties of the job Assessing patients with a wide variety of mental and physical health comorbidities using a holistic approach, considering all aspects including social, emotional, and spiritual wellbeing within the hospital environment. You would manage a caseload of people with complex needs in addition to providing clinical leadership, education and specialist advice for a range of other professionals, families, and carers. Our main focus is in District General Hospitals, but the service also extends to Community Hospitals. One of our aims is to prevent avoidable admissions for people with mental health problems, alongside in-reach into the various hospitals. Mental Health Liaison Service supports professional development and encourages all staff to take an active role in service improvement. This post is primarily for Older Adults, but there could be opportunities to add to the care plans of inpatients of working age too. You will be based in Carmarthenshire at either Glangwili General Hospital or Prince Philip Hospital, and you will be expected to work in community hospital sites in Carmarthenshire if required. Mileage allowance is payable, pool cars and a lease car scheme is available. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
