To deliver nurse led Screening clinics as directed by senior colleagues To manage a delegated caseload of patients To provide an outreach service by visiting patients at home, in hostels and a variety of temporary accommodation settings To ensure, at all times, that the needs of the patient are placed at the centre of service delivery Recognises, investigate and plans in consultation with the TB Nurse Specialist, the necessary action to resolve problems in relation to patient with adherence problem or likely to default treatment Providing support to patients facilitating regular clinic follow up and assist in locating patients atrisk of defaulting on treatment. Develop links and formalised referral methods with relevant services and to establish and maintaincollaborative care networks To undertake audit of relevant TB Service activities to ensure that standards for TB Infectionprevention and control are met To investigate contacts of TB index cases by delivering TB Contact Tracing in domiciliary andoutpatient clinic settings. Collecting laboratory data and clinical information Deputise in the absence of line manager To risk assess and develop individual care plans in partnership with clients based on a sharedassessment of need. To formulate and review/update TB service Policies as directed.
