Job overview
An exciting opportunity has arisen in Clatterbridge Cancer Centre on the Liverpool site for a motivated and proactive individual to become part of a busy and friendly Patient Flow team. The role can often be varied and complex but with a high level of job satisfaction.
The role requires the candidate to be a good team player and be able to work to tight deadlines with minimal supervision. The role will also offer the candidate opportunities to learn new skills within Acute Care Services Division and variety in their day to day activities.
The post holder will be responsible for liaising with staff to facilitate the safe and timely discharge of patients, with the support of the Patient Flow Team Leader and Co-ordinators, and be able to prioritise their own workload. Excellent administration skills, with a good knowledge of Microsoft applications and experience of working with patient records and using hospital databases and software, is advantageous.
In addition, be able to assist the Patient Flow Team in preparing information for the weekly Length of Stay meeting, prepare data collection for monthly reporting of Delayed Transfer of Care information, demonstrate effective and clear communication with patients and their families, keeping them involved in discharge planning at all times, and assist with the telephoning of patients post-discharge for a follow-up call.
The core hours of the department are 8:00am - 6:00pm.
Main duties of the job
* Actively demonstrate excellent written and verbal communication skills with all members of the multi-disciplinary team
* Respond to telephone enquiries and relay messages accurately, courteously and competently
* Work effectively as part of a team, supporting senior colleagues to implement agreed Discharge Plans
* Communicate effectively and sensitively with patients, relatives and colleagues
* Attend weekly Lengthened Length of Stay meetings
* Be present at the Daily COW MDT Board Round as required, and produce an action plan following attendance
* Be responsible for maintaining an accurate and up-to-date bed position for the Trust, emailing the information out as required
* Assist the Patient Flow Team Leader with audits, data collection, and review of bed utilisation, producing reports to demonstrate results (use of Excel)

Qualifications
* Knowledge and experience of using Excel and Microsoft Word
* Evidence of Professional and Educational Development
* GCSE – A-C in numeracy/literacy or equivalent

* Experience of minute taking and documenting appropriately
* Recognised IT qualification

Knowledge
