Wyre Integrated Network PCN are recruiting for a Practice Nurse to join their Enhanced Access team. The successful candidate will join a team of practices nurses, health care assistants, receptionists, GPs and ACPs offering evening appointments to patients within WIN PCN.
As this role is a bank role the successful candidate will be required to work across the 4 practices within WIN PCN. 2 Poulton-Le-Fylde practices and 2 Thornton-Cleveleys practices.
These include;
* Lockwood GP Surgery
* Queensway medical Centre
* The Village Practice
The Enhanced Access service provides out of hours appointments Monday - Friday from 18:30 -20:00.
Patients are able to book these appointments through their GP Surgery.
Main duties of the job
The post holder is required to provide excellent quality nursing care to NMC standard, to patients registered within the Poulton-Le-Fylde and Thornton-Cleveleys practices. These appointments can include bloods, wounds, vaccinations, contraception and routine screening.

* Venepuncture
* Cervical screening
* NHS Health Checks
* New patient health checks

* Assess, plan, implement and evaluate nursing care to meet the care needs of patients.

* Utilise EMIS web templates to record clinical data
* Sign post individuals to relevant organisations.

Person Specification
Qualifications
* Diplomas/ Degree in Long Term Management conditions
* Cytology screener
Skills, Knowledge, Competencies and Other
* Ability to remain calm under pressure.
* Competent and reliable
* Approachable
* Flexible
* Observant
* Knowledge of EMIS
Experience
* Substantial post registration experience in Primary Care/ Sexual Health
* Experience of autonomous practice
* Ability to take full responsibility for the clinical care of patients
* Good practical Practice Nurse clinical skills
* Commitment to multi-disciplinary team approach to care
* Experience using clinical computer systems: EMIS, DOCMAN
* Ability to manage Long term conditions
