This role will include working with the locality based Clinical Leads under the guidance of the Clinical & Strategic Lead. This PCN CYP MH Practitioner posts are a new service to support the clinical development and delivery of the Primary Care Networks (PCNs) in Essex. We are looking to recruit an applicant with expertise in the delivery of therapeutic interventions to children and young people (CYP) and their families to manage anxiety, low mood, and behavioural problems. PCN CYP MH Practitioners would be expected to hold a case load of young people with mild to moderate levels of complexity. The post holder will be supporting the development of the PCN's service in conjunction with the service manager and clinical lead. Working to facilitate wider service transformation within the local child mental health system. The post holder would be expected to work closely with the Service Manager and Clinical lead. They would also be expected to participate in the training opportunities available to them.The following are the core clinical responsibilities of the PCN mental health practitioner. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels: To work as part of a multi-disciplinary team in a patient facing role to assess and manage patients using expert knowledge To assist in the assessment of new patients with subsequent production and completion of individual care plans To provide specialist clinical support and advice to young people with mental health difficulties To work with patients to support engagement with appropriate services and treatment which may include onward referral to secondary mental health services or signposting to other mental health services, voluntary services and support groups To educate and involve family members and others in treatment as necessary To be responsible for the day-to-day planning of personal workloads To follow departmental policies procedures and guidelines To triage and also receive referrals and directed patients from within the PCN To communicate effectively with patients, recognising the need for varying methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating Flexible Working At NELFT, we realise that many staff have busy and demanding commitments outside of their working life and are unable to commit to the traditional shift patterns.
