Title Complex Needs Practitioner One Recovery Bucks (ORB) Salary: £26,544 - £28,640 Responsible to Service Manager A Work Sponsorship is not being offered for this role Summary Complex Needs Practitioners provide holistic support service for people with needs related to the impact of somebody elses substance misuse, including needs that arise as a result of familial substance misuse. The purpose of the role is to engage people in a variety of interventions, using a person-centred approach, empowering them to reduce risk, increase resilience and achieve positive outcomes. All practitioners work flexibly across the community and within our multi-agency partnership settings, with a central hub base at Aylesbury and High Wycombe. The caseload will include a mixture of community and multi-agency referral pathways including health, social care and justice. A core part of the role will be working with people within their families and community networks to address holistic support needs across all aspects of the persons wellbeing. This could include diverse work related to physical & emotional wellbeing, knowledge and understanding of drugs / alcohol / treatment, relationship with substance user, relationships with family and friends, meaningful activity, self-care and goal setting. Duties and Responsibilities Leading and being responsible for family and carer provision role within the team. To be a specialist resource within the team for the family and carer provision and interventions, to inclusion and to wider organizations within Buckinghamshire across a range of statutory and non-statutory multi-agency partners. To contribute to the development of family service protocols and policies in line with national guidance Support and build relationships with people, who have a variety of needs related to substance misuse, either their own substance use or somebody elses Hold a caseload and complete all recording and case management administration for your caseload accurately and promptly, adhering to GDPR and Data protection policies Develop co-produced, outcome focused support plans that reflect each participants personal strengths and goals. Support plans will be holistic, covering all aspects of the persons wellbeing. Ensuring that all family interventions and practice is evidence based and up to date with relevant national and local policy and legislation. Reporting this back to management and team. Supporting and training staff in developing the skills and knowledge specific to families and carers interventions. Attend and participate in clinical case discussions, completing agreed actions. To keep up to date with general health developments in the field of drug treatment particularly in their lead role of families. Client work To provide support, advice and interventions for service users including family members and carers, aimed at increasing choice and control in their lives, using a person centred, assertive outreach approach. Supporting people from the point of entry into our service, through their intervention and after-care, maintaining a full case load of family/carer clients and a reduced caseload of service users. To provide a range of interventions including CBT, brief solution focused therapy and motivational enhancement techniques. Providing screening, co-produced assessment, psychosocial interventions, integrated and outcome focussed care planning, after-care support, and onward transition to other services as required Identifying risk, completing and following risk management plans and reviewing them regularly. Identifying and responding effectively to safeguarding concerns. Making safeguarding referrals to the local authority and participating in all relevant multi-agency meetings. To create and facilitate group work interventions. Reducing drug and alcohol related harm to people within our local communities Advocating for the person to access partnership services; Working with people to support positive, holistic outcomes in relation to their health and wellbeing Follow the principles of the No Wrong Door Network to ensure that all people who use our service are enabled to access the correct support by referring or enabling access (including getting clients to appointments) to other services as appropriate. Act on advice and support from more senior staff as appropriate. General a) Maintain an up-to-date knowledge of developments in the field of alcohol, substance and an awareness of local community needs. b) Understand, uphold and work with the ethos, aims and objectives of Aquarius. c) Represent the organisation, raise its profile and promote its cause. d) Prepare for and participate in supervision and appraisals as required by the organisation. e) Identify own training and professional development needs and undertake training as required by the organisation. f) Attend meetings as required by the organisation. g) Work within Aquarius policies and procedures. h) Undertake such other duties as required by the organisation. i) Notify the Service Manager, Area Manager or Chief Executive of any occurrences which may affect the service, both at a professional and administrative level. j) Keep accurate records and the necessary data for reporting and evaluation purposes k) Provide written records and reports as required by the organisation. l) Travel throughout the geographical area covered by the service and for training courses, conferences and other events. m) To be flexible with regard to working patterns to meet the requirements of the service. Terms and Conditions There is a growing demand to provide a flexible service to meet client need and therefore there will be an expectation to work evening and some weekends, as required. The exact pattern of work will be determined by service need and may be subject to change. Holiday entitlement is 32 days and 8 Bank Holidays. The working hours are 37 hours per week however we will consider applications from those who wish to work on a part time basis. The salary scale for this post is £26,544 - £28,640. There is an expectation that postholder will start at the entry point. A pension scheme is available. All Aquarius staff is employed on an initial probationary period of 6 months. During this period, notice of termination by either party will be one month. An enhanced DBS check will be required for this post.
