**Community Specialist Social Workers - Operations, Mental Health, Learning\nDisability and Safeguarding Teams**\n\n**LB Hammersmith and Fulham \n£41,964 - £49,638 per annum**\n\n**Town Hall, King Street, London, W6 9JU \nHours per week: 36.00 \nContract type: Permanent**\n\nREF: LBHF619404\n\nCommunity Operations - The role holder will assist individuals, carers and\nfamilies to develop support plans to meet the outcomes identified, using a\nstrength-based, asset-based approach and family, friends and community options\nwhere appropriate and eligible needs are identified.\n\nMental Health - experience of mental health social work is preferred. There\nare opportunities to train as Approved Mental Health Professionals, (AMHPs)\nafter a qualifying period.\n\nSafeguarding - you will need experience of adult safeguarding, knowledge of\nrelevant legislation and making safeguarding personal.\n\nLearning Disability - To provide a professional social work service to adults\nwith a mental, health condition, their families and carers who are ordinary\nresidents of Hammersmith and Fulham in line with the requirements of the Care\nAct 2014.\n\nThe post holder will ensure that care and support are person centred and put\nthe people with whom they work in control of their lives.
