Job summary Forum Family Practice are looking to recruit a friendly, motivated, enthusiastic and experienced care navigator & prescription clerk for a minimum of 24 hours to join our team, due to increasing patient numbers. To be responsible for undertaking a wide range of reception duties and the provision of general support to the multidisciplinary team. Duties can include, but are not limited to, greeting and directing patients, effective use of the appointment system, booking appointments, the processing of information and assisting patients as required. To act as the central point of contact for patients, be responsible for the distribution of information, messages and enquiries for the clinical team, liaise with multi-disciplinary team members and external agencies such as secondary care and community service providers. To be responsible for the accurate and timely processing of prescription requests for the entitled population, liaising effectively with all relevant personnel. Main duties of the job Main duties of the job Ensure all patients and visitors are greeted professionally and politely with their queries dealt with effectively and efficiently. Answer all incoming calls from patients and other health care professionals. Divert or take messages as required. Deal with administrative queries and requests from patients & GP's. Maintain and monitor the practice appointments system. Make/re-arrange/cancel appointments as required. Initiate contact with and respond to requests from patients, other team members and associated healthcare providers Scan all patient information i.e. medical document sensoring all patient records are kept up to date. Deal with referrals to primary and secondary care Process and distribute incoming (and outgoing) post as well as dealing with other adhoc duties and general administrative tasks. Answer our prescription line taking requests for medications and dealing with patient queries Liaising with pharmacies and managing repeat dispensing tasks.
