Key Responsibilities: Work as part of a multi-disciplinary team (MDT) within the organisation. Assess and triage patients, including same day triage, and provide definitive treatment or make necessary referrals. Manage undifferentiated undiagnosed conditions, identify red flags and serious pathology, and take appropriate action. Use complex decision making to inform diagnosis, investigation, and management of episodes of care. Actively take a personalised care and population centred care approach to enable shared decision making. Complete relevant training for multi-professional clinical practice and CPD supervision. Advise patients on general healthcare, promote self-management, and signpost to social prescribing services and community resources. Perform specialist health checks and reviews, investigatory procedures, and collect pathological specimens. Support the delivery of anticipatory care plans and lead services like monitoring blood pressure and diabetes risk in elderly patients. Provide an alternative model to urgent and same day GP home visits for the network.
