An exciting opportunity has arisen for an experienced nurse to become the Clinical Team Lead of Halton Wellbeing, a community health service based at The Bridges in Widnes. The successful post holder will have both clinical and operational responsibilities and must be able to demonstrate strong leadership and excellent clinical skills to support and lead the team to deliver high quality clinical care.
We are looking for someone who is dynamic and motivated, and committed to providing visible daily leadership and clinical expertise to the team to ensure that the service provided meets local and national targets. The post holder will have highly developed communication skills enabling them to build effective working relationships with stakeholders and promote successful partnership working.
Main duties of the job
The post holder is responsible for the day-to-day running of the Wellbeing Service providing line management and clinical leadership to the team. Responsibilities include:
1. Coordinating and leading the team to provide health assessments for patients on the SMI register and ensuring that monthly targets are met.
2. Performance monitoring and management including engaging with the commissioner and service evaluators.
3. Working with the team to ensure that standards of patient care are high and to maintain safety and quality of the patient experience.
4. Using your specialist knowledge and experience to provide a high-quality nursing service for patients with SMI to assess and advise on physical health care including diet, weight gain, exercise, lifestyle, sexual health, and medication monitoring.
5. Leading and undertaking clinical audits.
6. Liaising with GP practices in Halton to ensure targets are achieved.

Job responsibilities
Job Purpose:
The post holder will provide professional management to and coordinate and participate in the clinical assessment for the Wellbeing Service in the borough of Halton. The post holder will facilitate access into the Wellbeing service and related Services in Halton. The post will involve the development of the service working across a range of health and social care teams leading to the provision of assessment in community and hospital settings. The post holder will also provide clinical managerial support to the Wellbeing Service workforce.
Duties and Responsibilities:
PRINCIPLE RESPONSIBILITIES:
1. Ensure the provision of an assessment service for the Wellbeing service and related services through the development, and day to day management of rota of professional staff.
2. Provide skilled holistic specialist clinical assessment, care and advice to people in their own homes and ensure appropriate care to meet assessed need.
3. Liaise across the range of health and social care services to ensure the timely provision of care and related services.
4. Undertake service monitoring, audit evaluation and review to ensure compliance with best practice and clinical governance requirements and the achievement and development of a range of associated outcomes and targets.
CLINICAL REQUIREMENTS:
1. Undertake comprehensive assessment of people to determine the appropriate provision of care.
2. Undertake a range of clinical treatments, designed to promote and maximise independence.
3. Provide clinical advice and support to a range of professional staff undertaking assessments as part of a rota.
4. Manage a caseload of patients and delegate the same appropriately.
5. Ensure timely and accurate record keeping within the service in accordance with professional, Trust and partners policies and procedures.
6. Work with other senior clinicians and managers in developing assessment within and related services.
7. Establish and maintain effective communication and interventions with patients; relatives and carers in complex and challenging situations.
LEADERSHIP REQUIREMENTS:
1. Lead the development of the Wellbeing service in Halton.
2. Support the Operational Manager in leading the development of Wellbeing Services within Halton.
MANAGERIAL RESPONSIBILITIES:
1. Ensure sufficient cover for the assessment service through management of a staffing rota.
2. Provide management support to clinical staff working as part of the rota.
3. Provide direct line management to the Trust employed clinicians within the assessment service.
4. Undertake Personal Development Reviews with directly managed clinicians as per the Trust Policy.
5. Coordinate patient care in appropriate settings with services following assessment.
6. Manage information from service providers in relation to capacity demand and supply and associated waiting lists.
7. Contribute to the data requirements of the service.
8. Adhere to and apply the various policies and procedures of the Trust and partners in relation to personnel, health and safety, complaints, data protection, access to health records and others as directed.
SERVICE DEVELOPMENT REQUIREMENTS:
1. Participate in reviews of the service to identify areas for improvement and further development.
2. Assist in the development of the project, action and development plans to achieve service improvement.

ANALYTICAL AND INFORMATION REQUIREMENTS:
1. Undertake and participate in regular audit and review of the service and report on the same.
2. Use national and local policy, data and information to support the development of the service.
3. Assist in the preparation of performance reports using service data.
CLINICAL GOVERNANCE REQUIREMENTS:
1. Monitor the efficacy and efficiency of the assessment service through the adoption of quality assurance mechanisms.
2. Ensure the robust procedures are adhered to for the identification and reporting of clinical and non-clinical incidents.
3. Support the development of staff through the use of Personnel Development Reviews, reflective practice mechanisms and team/service meetings.
4. Access regular clinical supervision.
5. Participate in research, audit and evaluation.
6. Promote a culture within the service that supports continuous development, learning and improvement.
EDUCATION AND TRAINING REQUIREMENTS:
1. Identify own training and development needs through personnel development planning and reflective practice.
2. Undertake Personnel Development Reviews for directly managed staff. Contribute to the same for other team members not directly managed.
3. Ensure staff make the best use of training and development opportunities afforded them and that such learning is shared across the service.
4. Ensure that all staff undertake mandatory training as per the Trust and partner policies.
COMMUNICATION/RELATIONSHIP REQUIREMENTS:
1. Liaise with patients, relatives, carers, acute and community clinicians, social care staff, voluntary/charitable and other non-NHS sector services.
2. Work with patients and carers to ensure that their values, beliefs and views are reflected in their rehabilitation management plan.
3. Encourage active participation in rehabilitation.
4. Ensure that treatment and rehabilitation is provided in a professional, accessible and supportive way.
5. Work with other professional and statutory groups involved in treatment and/or case/care management of the patient to ensure that there is consistent and high quality implementation of care.
6. Avoid duplication, delay or distress to patient.
7. Ensure that record keeping is consistent with the Trust and Professional standards.
LEVEL OF INDEPENDENCE/FREEDOM TO ACT:
1. The post holder is required to execute the clinical duties of this role without reference to others.
2. The post holder is required to execute the day to day management of the service without reference to others.
3. The post holder is required to report all untoward incidences of a clinical and non-clinical nature.
4. The post holder is expected to identify resource constraints on the provision of the service.
