Job Responsibilities:
1. Provide clinical support and direction to community nursing staff within the locality by sharing their expertise.
2. Work alongside the Operational Team Lead to ensure staff are competent in their role and that new staff are inducted and supported to reach the competency required for their role.
3. Implement evidence-based practice to ensure best patient care.
4. Provide enhanced clinical support and expertise to the patients within their locality.
5. Hold a Specialist Community Nursing Qualification.
6. Hold an Independent Prescribing qualification (V300) or be prepared to undertake it within 12 months in post.
7. Promote a service that prevents patients from having inappropriate or unscheduled hospital admissions and helps facilitate hospital discharge.
8. Ensure care in their locality is based on sound assessment and care planning processes.
9. Be I.T. literate to support staff transitioning to mobile working.
10. Positively represent the service both internally and externally, communicating highly complex information using informal and formal means to ensure people are fully informed of the service.
11. Ensure that appropriate statutory and mandatory training has been undertaken by all team members.
12. Be responsible for the retention, recruitment, induction, and personal and professional development of staff.
13. Monitor clinical supervision, induction, preceptorship, and development of staff.
14. Exercise leadership and judgment in the management of a Community Nursing Team to ensure good clinical practice and maintain high standards of care, reviewing staff practice at regular intervals.
