As part of the Community Mental Health Transformation Program we are looking to recruit an enthusiastic, caring and proactive practitioner to support adults with moderate-severe mental health problems such as anxiety, depression, OCD and psychosis. The successful candidate will work within our multi disciplinary team to hold a caseload of service users for time limited pieces of work. This work will be directly supported by Community Mental Health Nurses and other clinical practitioners. Your work will include supporting the physical wellbeing of service users. Main duties of the job Reducing social isolation/loneliness by supporting the person with social re-engagement Introducing the person to community resources Signposting and supporting people to groups within the community and accompanying them to groups initially to build confidence Supporting person to leave the house to increase confidence as part of graduated program To carry out anxiety management To identify and develop lists of appropriate community resources available within the area To introduce people to new hobbies /interests/ activities and link them with Life Rooms and other appropriate local provision To support the person to develop social networks To support the person to structure time/planning for week to increase engagement To help facilitate transfers back into primary care services and out of the Community Mental Health Team.
