About us
We are a team of around 25 who work closely together. We are based at Pilgrim hospital, Boston and Lincoln County hospital but also provide support to Grantham hospital. We work 7 days a week including bank holidays. We are recruiting to the team at many levels as the role of the team supports patient discharges and our colleagues on the wards. We work as part of the wider system in Lincolnshire with the community. We have introduced a hybrid model for a fully integrated discharge hub, which is very exciting. This will improve timely updates and provide a system wide triage team including Social care and Lincolnshire Community Health Services (LCHS). We are a progressive team not only updating how we work as a system wide team but also around our D2A model, which we are still developing further.
