See and Solve Senior Social Worker Grade 9

You will work as part of an integrated multi-disciplinary team in the See and Solve Team to provide high quality social work practice; ensuring individuals have a solution focused plan towards achieving their goals and maintaining their independence. You will work creatively with individuals, local communities and our partners, empowering them to deliver the best possible outcomes for people and to create a positive Adult Social Care culture.
We are looking for individuals who can demonstrate:

Ability to establish professional, effective working relationships with a range of partners/colleagues, adults, families and their carers
An individual who can support and help drive the new Discharge to Assess model once it is established.
You must be a qualified social worker and be registered with Social Worker England. This post will provide and support expert and effective social work practice in complex situations, assessing and managing high levels of risk and providing expert opinion in relation to decisions surrounding complex cases. You will undertake a professional leadership role taking responsibility for the learning and development of others, through leadership and management, supervision, mentoring, assessing and teaching.
See and Solve is the initial stage of the Adult Social Care Operating model and as and such we provide advice and information on how to prevent, reduce or delay the development of needs, about services, including universal and community services, eligibility criteria and how the system works including processes and the support available to make sure people have as much control over those processes as possible;
The team carries out their responsibilities to signpost, give advice and guidance, establish eligibility for adult social care support and assess under The Care Act. Senior Social Workers within this team have the added responsibility of responding to duty enquiries including visits in the community, allocation of work to Social Care Advisors and supervision of other staff.
