An exciting opportunity has arisen for a full time Band 6 care co-ordinator to join our team. We are looking for a versatile, motivated, committed and experienced mental health nurse to work with adults over the age of 65 who are living with a functional and/or organic illness.
We are a supportive, experienced and highly motivated multidisciplinary team. The successful candidate would be, committed to work within the recovery model and dedicated to improve the well-being of service users and their carers'. As a member of the team you will manage the holistic needs of people on your case load, undertaking a range of assessments, provide a range of psychosocial interventions. As a care coordinator you will also take responsibility for commissioning care using Local Authority systems and procedure.

Main duties of the job
To deliver evidence based holistic care to clients within the context of a multi- disciplinary, multi-agency Community Mental Health Service.
To take responsibility for a clinical caseload providing comprehensive assessment, care and treatment to clients referred to the Community Mental Health Team in accordance with Parts 2 and 3 of the Mental Health (Wales) Measure 2010.
Foster robust working relationships with all partners and key stakeholders, in particular Local Authority, Primary Care and the Third sector.
Contribute to the continuing improvement and development of community mental health services.
The ability to speak Welsh is desirable for this role Welsh and/or English speakers are equally welcome to apply.
