We have an opportunity for a secondment to the Early Help Case Management Team in Rushcliffe.
Position: 1 x 18.5 hours - Rushcliffe area, based at Home Brewery Building
We are looking for experienced children, young people and family practitioners who have a high level of personal drive and commitment and excellent interpersonal and decision making skills.
The main elements of this role include:
1. Acting as the lead professional in Early Help cases in line with service guidance.
2. Completing structured assessments and planning, commissioning, facilitating or delivering intervention plans, and undertaking reviews of children, young people, parents, carers, and families in line with service guidance.
3. Chairing multi-agency “Team Around the Family/Child” meetings.
4. Keeping timely and accurate records of work, including running records and the entry of data as specified.
