Job summary We are seeking an enthusiastic, proactive advanced clinical practitioner with experience working in the community. The role involves working within a community nursing team, supporting the team to offer a high level of care to patients in the place they call home. The post holder will be part of the Urgent Community Response service, visiting patients who are acutely unwell and assessing their clinical and functional needs to try to reduce unnecessary hospital admissions. They will also be involved in the development of the virtual wards, working closely with hospital colleagues. Hours per week: 30.00 Main duties of the job The post holder will use their skills within the four pillars of Advanced Practice to deliver a very high level of care to complex patients in the community. They will assess patients clinically, provide clinical leadership and service improvement and development through research and education.
