South Woodham Ferrers PCN is excited to offer a fantastic opportunity for a practice nurse to join our team on a part time basis, for 6 hours per week on a Saturday on a bank/locum basis, with the possibility of additional hours in the future.,
* Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
* Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
* Promoting general health and well-being, as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
* Managing and treating long-term conditions, in line with national and local policies and the practice needs.
* Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
* Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
* Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
* Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
* Meeting the needs of patients presenting for opportunistic wound care.
* Hypertension review and management.
* Running asthma, COPD and diabetes clinics and reviews.
* New patient and health checks.
The Dengie and South Woodham Ferrers Primary Care Network is a collaboration of seven GP practice, serving approximately 45,000 patients from the Dengie, Bicknacre, Burnham-on-Crouch and South Woodham Ferrers area.
