Take a lead role in the creation of capacity to meet demand and liaise with Discharge Lead and Matrons when insufficient demand is created. Participate in systems and processes that maintain and improve patient flows. Develop effective partnerships with clinical and clerical staff and an environment which fosters open communication and participation. Participate in developing effective partnerships with other CSUs and their Admission/Bed Management Teams that enables timely and appropriate patient placement. Contribute to the development of operational policies/clinical protocols that optimise patient flows on all sites. Facilitate referrals to primary care, social services or third sector organisations. Contribute to providing a service that maximises the use of community resources and facilitates the timely and appropriate assessment and/or admission of patients, for the GPs in Leeds. Maintain accurate records in accordance with the Trust guidelines. Report accidents, untoward incidents and complaints. Undertake duties on other wards or departments as and when required by service demands or patient need. Act in such a way that safeguards the health and wellbeing of children and vulnerable adults at all times. Is familiar with and adheres to, the LTHT safeguarding policies. Work closely with clinical teams and external agencies to support effective patient discharge. Promptly alerts the Discharge Lead of any delays or difficulties within a patients discharge plan. Promptly alerts Nurse in charge to difficulties in patient flow, placement or when capacity issues arise within their responsible ward areas. Collects and maintains accurate and up to date information regarding patient flows (acute and elective admissions, discharges, transfers, delays). Ensure all patient and carer enquiries are dealt with in a friendly and professional manner. Participates in daily MDT meetings to ensure seamless services are delivered. Raise concerns with regards to risk, danger, malpractice or wrong doing by following the LTHT Whistleblowing policy and supporting processes. Promote a positive learning environment where lessons are learnt from patient/carer complaints. Actively participate in the development of own personal development plan and appraisal Complete mandatory and priority training within the correct time periods.
