The Nottingham CityCare Urgent Treatment Centre (UTC) is a Clinician-led service that provides assessment and treatment to people with urgent but not life-threatening health problems.
These posts will suit enthusiastic, driven, motivated and flexible Registered HealthCare Professionals with demonstrable experience of working in an independent practitioner role in a first contact environment. The post holder will be required to engage in CPD including a mix of self-directed learning and mentored practice supported by internally and externally taught training.
Applications will be considered from those looking for full or part-time employment. There is a requirement for flexible 365-day working with shift works covering 0645am to 10pm daily.
As an autonomous practitioner, you will assess patients who present with urgent but not life-threatening illness and injury, plan their care, administer treatment on site, and discharge supported by senior colleagues where required. Where on-site treatment is not appropriate, you will refer to other healthcare professionals / external agencies.

Main duties of the job
The UTC is a training environment where employed practitioners will support the development of all disciplines of health care professionals including student nurses, pharmacists, GP SpR, medical students, and paramedics.
The post holder will manage an Urgent Care Caseload within a multi-professional clinician-led environment, dealing with a wide range of presenting undifferentiated conditions.
The post holder will work alongside a multi-disciplinary team of clinicians with acute and primary care first contact experience, including Nurses, Health Care Support Workers, Emergency Care Practitioners, Radiographers, and Paediatric Nurses to make up the team of the day working collectively in the assessment of undifferentiated urgent care patient presentations.
To work with the CityCare multidisciplinary team in the provision of primary healthcare to residents and visitors to Nottinghamshire.
To represent Nottingham City Care Partnership locally, regionally, and nationally if required to do so.

Job Purpose

The post holder will:
* Provide clinical assessment and deliver treatments to service users who have undifferentiated health care requirements in order to support the services delivered by the primary care team and prevent unnecessary acute hospital attendances.
* Utilise advanced clinical skills and competencies in health assessment, medicines management, leadership, and case management.
* Maximise patients' health and reduce risks that contribute to ill health, thereby reducing unnecessary admissions to acute services.
* Participate in the development of a specific clinical area e.g., Paediatrics, Sexual health, Safeguarding and be an autonomous practitioner who is open to the flexibility required for the role.
* Participate in training opportunities for self and others within the UTC.
* Support and deliver training to UTC staff, staff within CityCare, and the wider healthcare community.
* Provide mentorship and support to UTC staff, staff within CityCare, and the wider healthcare community.
* Provide mentorship, support, and training to students (including medical, nursing, and allied health professionals). Will take operational charge of a shift on a planned rota basis in the absence of the UTC Coordinator.

Clinical Responsibilities
* Comprehensively assess, review and evaluate the needs of service users with undifferentiated healthcare needs to improve their physical, psychological, and mental health wellbeing to promote self-care and reduce the need for hospitalisation.
* Accurately prioritise patients within the service, undertaking clinical assessment and providing the necessary treatment including the use of Patient Group Directions in line with local/national policies, procedures, and guidelines as appropriate.
* Carry out diagnostic tests only where clinically indicated in line with organisational, local, and national guidelines and appropriate to the Urgent Treatment Centre setting.
* Develop a plan of care in conjunction with the service user through: application of clinical knowledge and analysis of symptoms and data, identification of risk factors associated with those conditions, recognition of early signs of acute illness, involving the patients and the carers in the care plan and ensuring it is understood, documenting safety netting plans and processes.
* Utilise effective communication to aid onward referrals to specialists as appropriate, facilitating a coordinated primary healthcare response to service users' needs.
* Work in an autonomous manner without direct supervision whilst recognising the limitation of the practitioner role.
* Carry out enhanced clinical skills as required- this may include suturing and plastering.
* Keep accurate and immediate records of all interventions fully utilising software systems in place.
* Encourage clients to carry out self-care as appropriate through effective communication of health care using written information as appropriate.
* Advise clients on the most appropriate use of Primary, Secondary, and Emergency services.
* Maintain patient confidentiality as per Urgent Treatment Centre and CityCare's clinical guidelines.

Person Specification
Qualifications
* Registered healthcare professional, e.g., Nurse, part 1 of current NMC register, Paramedic, ECP, Clinical Pharmacist, Physiotherapist.
* Recognised teaching/mentorship qualification or willingness to work towards.
* Evidence of successful completion of university study (level 6 or 7) in a relevant subject.
* Chronic Disease Management Qualifications.
* Recognised Advanced Practice Qualification.
Experience
* Practitioner with experience of first contact care i.e., assessment of patient presenting with undifferentiated illness and injury, some of which should be in Primary care or Emergency Care.
* Knowledge and experience of multi-agency working.
* Understanding of legislation relating to clinical practice, and relevant professional standards and codes.
* Experience of policy development and implementation.
* Management experience and skills.
* Mental Health experience.
Special Requirements
* Ability to be flexible over hours worked within contracted hours to meet the needs of the service.
* Able to fulfil physical demands of post, this may involve moving and handling, fine motor skills are required for clinical activities such as suturing.

* Proof of vaccination & immunisation status in accordance with Department of Health guidelines.
