Hospital Discharge - Later Life Service - St. Helens
Are you an experienced and driven Social Worker with a passion for supporting older adults? The Later Life Service in St. Helens is looking for a Qualified Social Worker to join their team and make a difference in the lives of individuals aged 67+ with age-related frailty and mental health needs, such as dementia.
Role Details
Position: Qualified Social Worker (QSW) - Hospital Discharge
Location: St. Helens (out-of-borough travel required)
Pay: £35 LTD per hour
Hours: Monday - Thursday 08:45 - 17:15, Friday 08:45 - 17:00
(Condensed hours not available, but some flexibility is offered)
Car Driver: Essential (agile working required)
Responsibilities
1. Completing hospital discharges for out-of-borough hospitals, including:
2. Care Act assessments
3. Mental Capacity assessments
4. Best interest assessments
5. Attending case conferences as needed
6. Managing a busy caseload and a high volume of referrals
7. Undertaking Care Act assessments, devising care plans, and reviewing services
8. Applying knowledge of relevant legislation and government policies, including:
9. Mental Capacity Act (MCA)
10. Deprivation of Liberty Safeguards (DoLS)
About the Team
The Later Life Service is a large, dynamic team comprising 15 Social Workers and 15 Assessment and Review Workers. They focus on providing high-quality care and support to individuals with age-related frailty and mental health needs. The team offers:
1. Strong support structures
2. Regular supervision from Team Managers or Advanced Practitioners
What We're Looking For
Essential:
1. Qualified Social Worker with strong experience in adult social care
2. In-depth knowledge of relevant legislation (Care Act, MCA, DoLS)
3. Confidence in completing assessments, devising care plans, and attending case conferences
4. Ability to manage a busy caseload in a fast-paced environment
