Willows Health Group has an exciting opportunity for a Practice Nurse to join our committed, hard-working and multi-disciplinary team of clinicians. The post holder is responsible for ensuring they delivery of safe and effective nursing care to the whole practice population and accountable for nursing service delivery.
They will work closely with the Site Managers and GPs to deliver the practice priorities. Clinically, the focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions to all patients. As autonomous practitioners, the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making.
This role will additionally allow the post holder to work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership and direction for the nursing team.
They will also assist the CQC Registered Manager with duties relating to CQC outcomes ensuring compliance is maintained
Main duties of the job
Our Practice Nurse's assess, screen, treat and educate all sections of the community, from babies to older people. The successful candidate will provide nursing care to patients in a Primary care setting, supported by a team of experienced GPs, Advanced Nurse Practitioners, Allied Health Professionals, Specialist Nurses and Practice Nurses.

Job responsibilities
1. Health Assessments and Routine Screenings
2. Conduct comprehensive health assessments for patients, including taking blood pressure, measuring vital signs, and performing basic health screenings.
3. Assist in the identification of potential health risks and provide appropriate interventions or referrals as necessary.
4. Chronic Disease Management
5. Manage chronic disease clinics for conditions such as diabetes, asthma, COPD, and hypertension.
6. Conduct regular reviews and assessments of patients with long-term conditions, adjusting treatment plans and providing education to optimize self-management.
7. Immunisations and Vaccinations
8. Administer vaccinations and immunizations according to national guidelines and practice protocols.
9. Health Education and Promotion
10. Provide health education and promotion to patients on topics such as smoking cessation, weight management, nutrition, and exercise.
Support patients in making lifestyle modifications to improve their health outcomes and prevent disease progression.
Patient Care Coordination
Coordinate care for patients with complex health needs, liaising with other healthcare professionals and community services as necessary.
Collaborate with multidisciplinary teams to develop and implement care plans that address the holistic needs of patients.
Documentation and Record Keeping
Maintain accurate and up-to-date electronic medical records, documenting all patient interactions, assessments, and interventions.
Ensure compliance with data protection regulations and confidentiality policies in the handling of patient information.
Professional Development and Quality Improvement:
Participate in ongoing professional development activities to enhance clinical knowledge and skills.
Contribute to quality improvement initiatives within the practice, identifying areas for improvement and implementing best practices to enhance patient care.
Cervical Screening (Pap Smears)
Perform cervical screening procedures,
Obtaining informed consent, Explaining the procedure to patients,
Collecting cervical samples,
Ensuring sample quality, and
Providing emotional support to patients throughout the process.
