Main duties of the job
As a Clinical Pharmacist, you will play a key role in optimising medication use, improving patient outcomes, and ensuring safe and effective prescribing. Your responsibilities will include delivering structured medication reviews, supporting chronic disease management, and providing expert advice on medicines. The role also involves working across care interfaces, supporting care homes, and ensuring seamless patient care.

Job responsibilities
Within scope of practice:
1. Undertake clinics, diagnose, and treat patients. Manage various chronic diseases, including heart failure reviews, structured medication reviews (SMRs), hypertension, lipid management, and other conditions based on practice needs.
2. Provide pharmaceutical input for chronic disease management, ensuring effective contact and adherence to treatments to improve outcomes.
3. Carry out medicines optimisation reviews for patients in care homes and with complex conditions.
4. Identify patients needing medication reviews to optimise drug therapy, reduce polypharmacy, and minimize waste, communicating directly with patients as necessary.
5. Manage long-term conditions through structured Medicine Reviews and Chronic Disease Management clinics.
6. Review and act on hospital correspondence, particularly where medicine reconciliation or initiation is required, and manage medicines transfer of care, liaising with local pharmacies to improve outcomes.
7. Provide advice on queries from healthcare professionals.
8. Advise patients on medication side effects and interactions.
9. Support GP practice staff, practice patients, and the wider team with medicines and long-term disease advice.
10. Advise on risk management relating to prescribing and monitoring of specialist drugs, including controlled drugs and shared care arrangements.
11. Support the improvement of disease registers to ensure appropriate patient identification, treatment, and monitoring.
12. Assist GPs with managing the repeat prescribing reauthorization process, reviewing requests, issuing prescriptions, and flagging those needing review.
13. Ensure patients have appropriate monitoring tests when required.
14. Participate in multidisciplinary meetings related to prescribing and train healthcare professionals and staff on medicines usage.
15. Conduct prescribing audits, provide feedback, and implement improvements.
16. Contribute to achieving Quality Outcome Frameworks (QOF) objectives.
17. Prepare evidence-based resources to support rational, cost-effective prescribing.
18. Support practice projects such as MOIS and others outlined annually by the practice and local ICB.
19. The successful candidate should have completed the CPPE Primary Care Education Pathway or plan to complete it within the current year.

Qualifications
* Mandatory registration with the General Pharmaceutical Council.
* Current DBS check.
* Knowledge of GPs and general practice services.
* Understanding of primary care prescribing, rational prescribing concepts, and improvement strategies.
* Excellent interpersonal, influencing, and negotiating skills.
* Strong written and verbal communication skills.
* Ability to communicate complex information understandably to patients.
* Proficiency in work-based IT programs.
* Ability to analyze complex technical information.

* Ability to gain acceptance and influence colleagues and patients.
* Experience documenting patient care on Emis or similar software.
