An exciting opportunity has arisen for a dynamic and enthusiastic trained nurse to join our established Coventry & Warwickshire Community TB Service. You will be working as part of the multi-disciplinary team providing high quality TB care in collaboration with other services within both the acute and primary care setting to provide a seamless service for people receiving TB treatment, their contacts and the wider community in accordance with nationally and locally agreed guidelines.
This is a fantastic opportunity for a suitable candidate wanting to develop their TB knowledge and to develop skills to become an autonomous specialist practitioner. You must have the ability to work both autonomously and as part of a multi-disciplinary team. The successful applicant will have their own case load of patients and will run nurse led TB contact tracing, BCG vaccination and New Entrant Screening clinics. Excellent communication skills and an ability to work with people with diverse health and social needs are essential.
The post holder will work as a member of the Community Coventry & Warwickshire TB Service contributing to the delivery of an effective TB service assisting in the diagnosis, treatment, prevention and control of TB.
• Works collaboratively with the multi professional team to develop and maintain a strategy relating to the care of patients in specialist area.
• Works both as an independent practitioner, and as an effective member of the wider interdisciplinary clinical team. Demonstrates excellent communication ability.
• Utilises an agreed service plan as a framework for service delivery.
• Uses data from audit / quality monitoring/ patient satisfaction questionnaires /service review to inform clinical governance. Utilises information to ensure that services are responsive to patient needs. Ensures patients are consulted in the development of services.
• Is responsible for ensuring own clinical practice complies with Data Protection/ Confidentiality/ Caldicott principles.
• Assists in maintaining standards of cleanliness in the clinical area and for a well maintained ward / department environment.
• Actively promotes diversity, including race diversity, both in the delivery of the Directorate’s services and in the management and development of staff within the clinical area. The post holder will work across acute Trust/ICB settings, as an expert in clinical field to deliver specialist clinical services to defined patient/client group. They will assist in the development of the service in line with national and local targets and provide specialist, interdisciplinary education and training to other health care professionals & those working locally in relevant services.
The post holder will be supported through an in-house training programme to achieve clinical skills and knowledge competencies as outlined in the CNS job description. Once in-house training programme completed and competencies achieved the post holder will be able to:
Have their own case load of active and latent TB patients and will be involved in nurse-led TB contact tracing, BCG vaccination and New Entrant TB screening clinics. The post holder will provide professional advice to other professionals, multidisciplinary team, patients and their carers. The post-holder will act as a resource for education and research.
• Holds own defined caseload.
• Communicates complex, sensitive information effectively and empathetically to patients and relatives. Maintains personally generated observations in records.
• Assesses, develops and implements evidence based specialist nursing care programmes, and advice for patients and relatives.
• Work as an expert practitioner in given area, acting as a resource for other interdisciplinary healthcare professions. Maintains a high profile in the clinical area, acting as a role model for good practice.
• Works with professional groups and other agencies in both primary and secondary healthcare settings. Assumes a central role in problem solving and service co-ordination to ensure patients and carers receive high standards of seamless quality care.
• Works with non-specialist interdisciplinary practitioners to compliment their care, and the quality of the service provided.
• When appropriate, participates in the establishment /running of nurse-led clinics. When trained, undertakes diagnostic and therapeutic procedures pertinent to specialist role.
• Actively participates in interdisciplinary meetings.
• Acts on clinical incidents pertinent to area, to reduce clinical risk.
• Provides informal support and education to patients/carers and all professional disciplines
• Participates in the delivery and evaluation of formal education programmes related to speciality.
• Identifies and plans education, participating in curriculum planning and delivery.
• Conducts/facilitates primary research/ contributes to research programmes. Promotes evidence based practice in specialist area. Is aware of clinical trials that may affect patients in own clinical area.
• Provides formal mentorship and / or clinical supervision as required.
• Contributes to the development of care pathways when necessary.
• Develops/ contributes to an information database of support services available to health care professionals and patient group.
• Uses research findings critically to develop practice.
• Monitors and maintains the health, safety and security of self and any practitioners he/ she is responsible for in work area. Identifies and assesses risks in work activities, and develops strategies to manage these effectively. Practices within risk management/ occupational health legislation and Trust procedures.
• Develops own skills, taking responsibility for continuing professional development and performance, maintaining own portfolio in accordance with post registration requirements. Works to achieve competencies appropriate to grade. Identifies own development needs in relation to current practice and future plans, setting personal development objectives.
• Establishes local and national networks, linking with other practitioners in specialist area in order to share/ learn from examples of good practice.
• A full and valid UK driving licence with daily access to a car for work purposes is essential for this post.
