We are looking for an experienced Nurse to take on team leader responsibilities of one of our team localities.
Working under the clinical leadership of the Associate Director of Patient Care and Communities, you will adopt a person-centered, coaching approach to the leadership of a locality team, fostering self-coaching, team working, self-awareness, personal growth and professional competence. With responsibilities for line management of the multi professional locality team and ability to deputise when needed. A minimum of 60% of time will be spent in direct clinical care.
You will ensure clinical practice is reflective of current developments in Specialist Palliative Care and End of Life Care. Promote a learning environment and participate in the development and delivery of educational programs.

Princess Alice Hospice is one of the top ten adult hospices in the UK, providing specialist palliative care and support to a community of more than a million people living in Surrey, south west London and Middlesex.
Our Hospice at Home service brings the skills and expertise of our highly trained and dedicated specialist team directly into our patients homes. This allows patients, their families, and carers access to the vital support they need at home, when time matters most.

To deputise for the Associate Director Patient Care andCommunities, supporting the strategic delivery of all services pertaining tothe community.
To submit statistics and activity reports as required.
To liaise frequently with the members of the team to ensurethat patients on the caseload are reviewed and supported according to thecaseload management guidelines.
To support the Associate Director Patient Care andCommunities to implement the Princess Alice Hospice Clinical Strategy andService Plan.
To receive and disseminate information through chairinglocality team meetings and attendance at other relevant meetings.
Supports the delivery of activity by clinical volunteers whowork with the Community team ensuring integration and support needs are met.
Participates in the recruitment, selection and retention ofstaff, to ensure that adequate staffing levels and skill mix are maintained,and that staff receive appropriate induction and personal development plans.
Supports multi-professional caseload management for allpatients, ensuring that all necessary information on patients is available asappropriate.
Under supervision and leadership by the Associate DirectorPatient Care and Communities contribute to the Hospice Governance arrangementsand apply Clinical Governance and Risk Management standards ensuring thatpolicies and guidelines are available and adhered to.
Clinical
Participates and takes a lead role in multi professionalteam meetings to provide informed specialist practitioner opinion and caremanagement options.
Will spend a minimum of 60% of the working week in direct contactwith patients and families
Promotes and supports the team to deliver a seamless packageof care, including liaison with external agencies, to enable patients to bemanaged in their preferred place of care
Works alongside the Nurse Consultant develop advancedpractice within the team, developing a team that is responsive and skilled inphysical assessment and non -medical prescribing, ensuring that any advicegiven is within the scope of practice
Under the supervision of the Associate Director Patient Careand Communities and the Nurse Consultant develop clinical leadership for theHospice at Home Team throughout the patient pathway from referral, triage,advance care planning to preferred place of care and death at the end of life. To participate in cover and the 24 hour on-call rota as appropriate, thatcovers the Princess Alice Hospice catchment area.
Educational
Keeps abreast of current developments in SpecialistPalliative Care and End of Life Care, and the implications they may have toclinical practice.
Acts as a role model and mentor, providing informaleducation and support to colleagues in their provision of care.
Promotes a learning environment for students, other clinicalplacements and visitors to the Team specifically and the Hospice generally.
Participates in the development and delivery of formaleducational programs internally and with external clinical and academicpartners.
Participates and leads as appropriate in developing andauditing nursing specific and multi professional standards.
Participates in and supports nursing specific andcollaborative clinical research, and leads in the development of evidence basedpractice.
Professional:
Ensures own Professional registration is valid and currentand adheres to own code and scope of professional practice.
Ensures that a professional portfolio is kept and updated inaccordance with revalidation as appropriate

Acts as an ambassador for the Hospice at all times,including representing the Hospice in fundraising and public relationsactivities as requested.
