About the Role:
CPL Healthcare UK is recruiting a dedicated and compassionate Care Coordinator for a domiciliary care agency based in Tunbridge Wells, Kent. The agency specializes in providing high-quality care to elderly individuals, as well as supporting clients with disabilities and palliative care needs. As the Care Coordinator, you will play a vital role in ensuring the effective and consistent delivery of care services to approximately 60 service users in their homes. You will work directly with care staff to oversee care plans, provide hands-on support when needed, and manage emergencies as they arise.
Key Responsibilities:
Care Coordination:
* Develop, implement, and review personalized care plans for service users.
* Ensure that care is delivered in line with each individual’s care plan, maintaining a high standard of care.
* Coordinate care staff schedules to ensure service coverage for all shifts and care requirements.
* Maintain effective communication with service users, their families, and healthcare professionals to ensure continuity and quality of care.
Direct Care Support:
* Provide hands-on assistance with personal care, mobility support, and daily living activities when necessary, particularly in emergency situations.
* Support elderly, disabled, or palliative clients with their individual needs to enhance their quality of life.
Staff Management and Support:
* Assist in the recruitment, training, and induction of new care staff.
* Provide ongoing supervision and guidance to ensure that care staff are delivering high-quality services according to the care plans.
* Conduct regular checks to ensure care standards are being met.
Emergency Response:
* Be available to respond to urgent care needs, including stepping in to provide direct support to service users during emergencies.
* Troubleshoot and resolve any immediate issues with care delivery in a timely manner.
Record Keeping and Reporting:
* Maintain up-to-date and accurate records of care plans, service user progress, and any incidents or changes in conditions.
* Report any concerns or changes in service users' health or circumstances to relevant healthcare professionals and management.
Travel:
* Travel to service users' homes within the local area to monitor care provision and support staff, including providing direct care in emergencies.
Requirements:
Experience:
* Previous experience in care coordination, management, or a supervisory role in a domiciliary care setting is highly desirable.
* Experience in elderly care, palliative care, or care for individuals with disabilities is preferred.
* Familiarity with care standards and regulatory requirements, such as CQC guidelines and care planning processes.
Skills and Attributes:
* Strong communication, organizational, and problem-solving skills.
* A compassionate, empathetic, and patient approach to care.
* Ability to work effectively both independently and as part of a team.
* Capable of managing emergencies and delivering hands-on care as needed.
* Proficient in care management systems and documentation.
Other:
* Must have a valid UK driving license and access to a vehicle for travel.
