An exciting opportunity has arisen for a community rehabilitation band 6 Physiotherapist to join a supportive and enthusiastic community ward therapy service, delivering rehabilitation to patients in a community hospital setting at Clitheroe Hospital, or on the Intermediate care wards at Burnley General Hospital.
The team is a skilled multidisciplinary team and supports improvement and learning.
You will be working with patients with a large variety of conditions and disabilities. You will be responsible for assessment, intervention, and discharge of a caseload of patients, act as the direct supervisor to staff and provide support and expertise to the other members of the team.

Clinical supervision and working alongside a clinical service lead to support your development.
The service operates across 6 days Monday-Saturday.
Main duties of the job
The post holder is an autonomous practitioner, responsible for provision of specialised assessment and treatment of patients referred to the service. Responsible for managing a caseload of patients with a variety of complex conditions including long-term condition management and hospital discharges. You will carry out specialist assessment and treatment, rehabilitation and onward discharge to community Therapy services.
Work across the MDT as part of the ward team.
Provide support to a rotational band 5 Physio and other members of the MDT.

Person Specification
EVIDENCE OF
* Post graduate therapy experience to include community and rehab.
EVIDENCE OF
* Profession specific Diploma/degree in Occupational Therapy or Physiotherapy.
* Health Professions Council Registration.
* Evidence of CPD maintained in a portfolio including attendance at recent postgraduate courses relevant to the clinical fields in rotations.
* Membership of the Chartered Society of Physiotherapy/ Royal College of Occupational Therapy.
* Involvement in Special Clinical Interest Group.
EVIDENCE OF
* Post graduate therapy experience to include community and rehab.
* Post graduate training in relevant clinical areas.
* Experience of delivering training to colleagues in health and/or other agencies.
* Experience of working in an integrated therapy or multi-professional MDT.
* Experience of therapy student and/or support staff supervision.
* Contribution to clinical education of less experienced staff/students/assistants.
* Experience of working with patients with dementia.
