The D2A (Discharge to assess) Clinical Case Manager role sits under the Surrey Heartlands All Age Continuing Care (AACC) Team and works closely and collaboratively within the immediate team and external agencies, building working relationships with all including the patient and their families and carers. Primarily the role involves the assessing of individuals for CHC eligibility, writing of a robust and comprehensive assessment, liaison with multi disciplinary agencies and the gaining of all relevant information and evidence in support of the final recommendation.
The D2A Case Manager will have access to support and training in all aspects of Continuing Healthcare (CHC) National Framework in order to improve their knowledge and skills.
This post is suitable for an experienced Band 5 looking to progress their skills on a development pathway or an established Band 6 who wishes to specialise or gain experience in the Continuing Healthcare setting.
Main duties of the job
Once a patient is assessed as being eligible for NHS Continuing Healthcare (CHC) D2A pathway, (working with the Clinical Liaison Practitioner), or for CHC Fast Track pathway (and working with the Fast Track Clinician). Your role is to liaison with the patient, family and hospital staff and start gathering information with regards to the patient's care needs, and feed back to the relevant AACC clinician. You will be checking that the care in place is appropriate and safe, and you will be responsible for arranging attending the assessment for eligibility to NHS Continuing Healthcare funding.
You will use your existing and new knowledge and clinical skills to ensure that outcomes are determined in a consistent manner to ensure effective application and interpretation of the National Framework for NHS Continuing Health Care and NHS Funded Nursing Care.
You will work closely with the AACC teams, hospitals, community services and care homes to maintain and improve systems and processes to facilitate a smooth referral, assessment and review process to support patients. You will escalate complex issues to senior managers for timely query resolution.
Visits to the hospital are also required sometimes, to see the patients and gather clinical information before they are discharged. Ideally you will live within the Surrey Heartlands geographical area, as you will be expected to travel across Surrey for assessments and travel to your induction and meetings as required by the service.
