Family Navigator (E6563) - The Bridge Project
The Bridge Project is a well-established, vibrant Charity with a dedicated team providing services to a broad range of people experiencing the effects of deprivation and health inequalities. We now have a full-time vacancy for a Family Navigator based at Shipley Owlet Family Hub.
We deliver our successful Navigation Model with a small cohort of referred families to the team, whilst working closely with colleagues within the Children and Families Trust.
The team works with families who are primarily impacted by substance use and who are experiencing simultaneous additional issues linked to unmet needs and deprivation across a range of domains. This may include poor physical health/mental health, family court involvement, domestic abuse, chronic housing issues, exploitation, financial issues, and criminal justice involvement.
Our Navigation Model champions a high level of multi-disciplinary interagency cooperation and joint working relationships with families. We deliver an evidence-based, staged delivery model using trauma-informed approaches that offer intensive in-reach within the home. Navigators are skilled at rapidly building trusted relationships and providing practical support for individuals/families. Navigators set up, coordinate and ensure delivery of individualised support packages that access services/interventions families need to build strength, resilience, overcome challenges, gain confidence, and acquire the personal and social assets to meet their aspirations that positively impact their lives. There is a clear aim to support individuals to move to full independence of the service.
We are looking for someone who is qualified to Social Care Level 3 or equivalent or qualified through appropriate life or job experience, along with at least 2 years’ experience successfully delivering support services to people experiencing multiple need/multiple disadvantage. You will be highly organised with excellent communication skills and able to manage your time effectively, working as part of a supportive team to ensure a high-quality service is maintained. It is essential that you have experience of working with/or managing high-risk cases therapeutically and conducting person-centred and strength-based assessment of need/creating mutually agreed action plans and risk management plans.
Most of all, you will place individuals at the heart of your interactions, with the ability to build up a rapport and trust, with personal skills around warmth, compassion, empathy and resilience, with a desire to help people and maintain a supportive relationship. Often situations can sometimes be difficult and challenging so you will need to be non-judgemental and tenacious in order to deal with situations calmly, tactfully, responsibly and safely.
The successful candidate will be able to work 09:00 to 17:00 Mondays to Fridays, with an element of flexibility. It is essential that you have a driving licence with access to a vehicle, in order to travel anywhere in Bradford MBC, in the delivery of direct family navigation support services including clients’ homes. A willingness to undergo an enhanced DBS check is also a requirement of the post.
