We are looking for enthusiastic, forward-thinking registered nurses with a keen interest in Diabetes Mellitus to join the diabetes specialist nursing team at University Hospitals Plymouth NHS Trust as a Diabetes Inpatient Specialist Nurse.
The diabetes inpatient nursing team work 7 days per week (including weekends and bank holidays) to provide expert diabetes nursing care to inpatients at University Hospitals Plymouth NHS Trust and their families/carers. We deliver education and training in diabetes care to other healthcare professionals within the trust.
The Plymouth Diabetes Service comprises of specialist Doctors, Dietitians, and Nurses who provide inpatient services, perioperative and outpatient clinics, and group education to people with diabetes.
Main duties of the job
* To support and educate Health Care Professionals at University Hospitals Plymouth NHS Trust.
* To act as an expert resource, and facilitator to educate and train other healthcare professionals across divisional and organisational boundaries.
* To provide expert knowledge across UHP for the management and safety of those with diabetes whilst an Inpatient.
* Perform audit and research projects and develop policies and procedures to develop the service as required.
* The post holder will work within the inpatient team to manage a varying weekly caseload of approximately 5-7 referrals to each team member per day. Referrals are generated from ward staff, consultants, and review of the blood glucose monitoring systems.
* The team delivers education and training to staff in formal teaching sessions and on ad hoc ward visits.
* Other duties include developing and introducing comprehensive clinical care pathways in conjunction with other members of the Diabetes team and attending steering groups.

* Utilises advanced skills of assessment, interprets complex psychological, physiological, emotional, spiritual and cultural needs.
* Enables patient choice and involvement and initiates appropriate action.
* Co-ordinates inpatient care pathway and liaises across organisational boundaries.
* Plans specialised programmes of care and provides highly specialised advice to inpatients, relatives/carers and staff.
* Ensures each inpatient is placed in the correct treatment pathway at the appropriate time.
* Provides clinical advice and support to healthcare professionals within and across organisational boundaries.
* Develops and plans specialist nursing service provision.
* Refers to other disciplines/professionals unsupervised.
* Reviews inpatients independently and initiates changes in management of care.
Education
* Develops specialist educational and training programmes for healthcare professionals.
* Works collaboratively with local education providers including the University of Plymouth.
* Provides clinical supervision and mentorship to junior or new clinical nurse specialists / nurses / medical staff as required.
Management
* Supports development and implementation of policies, protocols, and procedures in accordance with local and national standards.
* Manages own caseload and coordinates inpatient journey across divisions and organisational boundaries.
* Works in close collaboration with consultants across organisational boundaries.
Person Specification
Qualifications
* Registered Nurse with current NMC registration.
* Diabetes module at degree or masters' level or working towards this or be prepared to study at this level.
Knowledge and Experience
* Demonstrable post registration experience of working with people with diabetes.
* Demonstrable experience of handling complex relationships.
Aptitude and Abilities
* Reliable, high standard of nursing practice.
